Collected after acceptance:
 

1. BOLT Medical Forms – this form will be used by administration to determine eligibility for participation in the trip, as well as responding to incidents that occur in the field. The med forms that BOLT collect come in two formats: a full medical form which captures as much of your medical history as possible. This form is kept at basecamp to ensure that we have as much information as possible in case we need to respond to situations in the field. We also collect a field medical form, which includes consolidated information that your trip leaders can easily reference in case they need to access the information quickly in the field. All of this information is stored on a secure Google Drive and deleted once the trips have returned from the field.
